At Your First Visit
***Please arrive 15 minutes early to complete your paperwork before your first appointment.***
When you enter the front door you will see a table with intake forms. Take one and complete the form and be sure to sign it. If your insurance carrier has sent you an authorization letter, attach a copy for our records. Your clinician will be up front shortly to get you for your appointment and they will collect your co-payment or amount you are responsible for today. There is not a receptionist to assist you unless it is an emergency, please wait for your clinician to come for you.
